How do I create an online form?

Online forms are created in a three-step process.

Step 1: Describe the Form & Set Rules

If you already know the template you want to use, select that first. Learn more about templates.

Give the form a title and description that explain what the form is about.

Set an answer to How many times can users fill the form?

  • "No limit" allows the user to submit the form as needed. The number of submissions is not capped.

  • "Fill max once" allows the user to submit the form one time only. If the form is requesting parent to select a student name, in that case, a parent can fill out the form one time per each student.

Set Require parent to select a student.

  • The online form a parent fills out may primarily regard student information. When this setting is enabled, the first form field is automated to ask a parent to select their child's name from the drop-down.

  • If the parent has one child attending the school, the form field shows only one name.

  • If the parent has two or more children attending the school, the drop-down shows the student names and will ask a parent to select the student the form regards.

Set Do you allow users to go back and edit the answers after submitting the form?

  • You have an option to allow or disallow edits after a parent has submitted the form.

Add Form Admins (besides you)

  • The Form Admins are notified once a day about new form submissions.


    Click Continue


Step 2: Add Form Fields

Click + New Field button.

  • Select the form field type (link)

  • Add data

  • Click Add

Use the up and down arrows to move the form field up or down.

Use Edit to adjust the text.

Use Copy to duplicate the form field.

Use Change Question Type if you want to adjust the field. Note: add the field first.

Click Save & Continue

If you need to leave the page and return to it later, clicking Save keeps your work and saves the form as a draft.


Step 3: Review and Publish

Review your settings one more time.

  • Click Show form fields to review the form

  • Check the setting for How many times can users fill out the form?

  • Check the setting for Require parents to select a student

  • Check the setting for Do you allow users to go back and edit the answers after submitting the form?

  • Check the setting for Form Admins (you can leave the field empty, too).

Set the Audience

Choose the audience that needs to access and fill out your form. You can also choose a Group.



Opt-in to Announce the Form

Select if you want to announce the form in the School Feed upon publishing the form.

Set the Form's Type and Visibility

  • 'Private' online forms are displayed on the Online Forms page for parents and/or teachers.

  • If you don't want to include the form on the Online Forms page and just want to give a link to a smaller subset of users, you can set the form's visibility as 'Private unlisted.'

  • If you want to publish the form as a template, select 'Template.' Template forms are not visible to parents.




Click Publish.
Click Save as Draft if you are not ready to take the form live yet.
