The form field types are:
Paragraph
Use a paragraph to enter descriptive text into the form. This field does not collect any data.
Single-line text
Use this field for information that can be entered with just a few words.
Multi-line text
A multi-line text field creates a larger field for the user to enter information.
Select-one: Dropdown
Add at least two options. Users can select one option.
Select-one: Radio buttons
Add at least two options. Users can select one option.
Select-many: Checkboxes
Use checkboxes if you don't want to limit the answer to just one option.
Date: Calendar
Users can select a date from the calendar. No time selection.
Date: MM/DD/YYYY
Users enter a date in a standard U.S. format.
Time
Users select hour and minutes. The minutes are provided in five-minute increments. Users select AM or PM.
Date + time
Date + time is a combination field that collects both data and time. You can choose between calendar and MM/DD/YYYY fields for the date.
Range slider
Use the range slider to allow users to set their answer as a value to your question. A typical use-case example is a Survey.
You can set the slider with a minimum and maximum value. The minimum is as low as the user can give the value in the slider, and the maximum is how high a value can be.
Step defines the increments that the slider goes up or down. If you add 1, the user can set the slider in 1 increments. If you set 10, the user can set the slider in 10 increments.
The default is the value to which the system places the slider. For instance, if you collect answers between 1 and 4, you may want to set the default value to 2.
Use the question space to clearly define what your values mean.
File upload
Allow users to upload images or images and documents. Set the number of allowed files, too.
